1. Click Menu > Sales & Marketing > Manage Surveys.
2. On the next page, click the gold "Add a Survey" button
3. Title your survey and write a brief introduction
4. Below those fields, click the gold "Add a Question" button
5. Select which type of question you would like to ask, enter the question, and click the gold "OK" button; repeat this for as many questions as you would like to ask
6. You can edit, reorder, and delete your questions along the right of the list; view a preview of your survey by pressing the gold "Preview Survey" button
7. Once you have added your questions, add a title and contents to the Thank You page that customers will see once taking the survey
8. Click the "Back" button to lock your survey in place for future use.
